Plan Your First Visit

We're excited to start this journey with your family

Step 1

Before You Arrive

Complete intake forms (link to download)

Gather insurance card and referral (if required)

Bring any medical records or previous evaluations

Note current medications and assistive devices your child uses

Set up your patient portal to view appointments, clinical documentation, payment options, store credit card, and pay balance

Step 2 (10-15 minutes)

Check-In

Arrive 5-10 minutes early to complete final paperwork with front desk

Front desk will present final paperwork and verify insurance

Have ready: parent ID, patient's insurance card, and credit/debit card if copay applies

Allow your child to get familiar with the clinic environment

Step 3 (30-60 minutes)

Evaluation

Therapist interview with parent/caregiver about child's history and goals

Standardized assessment and observation

Play-based activities to observe movement, communication, or daily living skills

Parents are required to remain present in the room for the full evaluation

Step 4

Results & Treatment Plan

Therapist discusses findings and recommendations in person or via phone

Together, you'll set therapy goals and create a treatment plan

Schedule is created (typically 1-2x per week)

Therapist provides home activities and parent coaching to support progress between sessions
