Effective date: September 29, 2026
These Terms of Service (also referred to as our Terms and Conditions) set out the rules for receiving care from, and using the website of, Leaps & Bounds Pediatric Therapy, Inc. Please read them carefully.
1. Acceptance of These Terms
These Terms of Service (“Terms”) are a binding agreement between you (“you,” the patient, or the parent or legal guardian of a minor patient) and Leaps & Bounds Pediatric Therapy, Inc. (“Leaps & Bounds,” “we,” “us,” or “our”). They apply when you schedule or receive services, use our patient portal, or use our website at leapsandboundspediatrictherapy.org. By doing any of these, you agree to these Terms. If you are agreeing on behalf of a child, you confirm that you are the parent or legal guardian and have authority to accept these Terms for your child. Some policies are also provided to you as separate forms that you acknowledge; those forms and these Terms work together.
2. Services We Provide
Leaps & Bounds provides pediatric physical therapy, occupational therapy, and speech therapy for children from birth through 21 years, including the option of hippotherapy as a treatment tool within those disciplines. Care begins with an evaluation, which is an assessment used to determine whether ongoing therapy is recommended. We do not provide behavioral services such as Applied Behavior Analysis (ABA), and we are glad to help point families toward appropriate providers.
3. No Guarantee of Outcomes; Not Medical Advice
Therapy outcomes vary from child to child. While our licensed therapists use evidence-based practices, we cannot and do not guarantee any specific result. Nothing on our website or in our communications is a promise of a particular outcome. Website content is for general information only, is not medical advice, and does not replace evaluation, diagnosis, or treatment by a qualified provider. If you believe your child is experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
4. Referrals and Authorizations
Most insurance plans require a referral or prescription from your child's doctor before therapy can begin, and the referral must list your child's primary care doctor. Without a referral that includes the correct primary care doctor, we cannot begin the process to add your child to our waitlist. If your child's insurance requires authorization, a change in medical group or primary care provider may require a new authorization, which in most cases the new provider must submit before services can continue. This process can be time-consuming and may cause delays.
5. Appointments and the Waitlist
Once your child's forms are complete and insurance and referral are verified, your child is added to our waitlist. The typical wait is up to 120 days before an evaluation. Recurring appointment availability varies by discipline. You can confirm, change, or cancel appointments through your patient portal or by contacting our Front Desk at FrontDesk@LeapsPediatric.Org or (951) 340-0431.
6. Attendance Policy
Consistent attendance is essential to your child's progress and to keeping recurring appointment times available. By receiving services, you agree to the following:
Appointments may be canceled up to two weeks in a row. A third consecutive week of cancellations will result in weekly scheduling going forward, and the recurring appointment time will be removed.
If fewer than 60% of appointments are attended within a 30-day period, the recurring time will be removed and you will be asked to schedule weekly.
Failing to attend a scheduled appointment, or failing to call before the appointment begins, is not permitted. Two occurrences will result in removal from the recurring schedule.
Cancellations for vacations, specialist appointments, or preplanned activities must be made at least 24 hours in advance.
The patient must be present for all appointments, both in clinic and via teletherapy.
Grace period: 10 minutes for 30-minute sessions, and 15 minutes for 45- or 60-minute sessions. If the patient arrives after the grace period, the appointment will be rescheduled and the patient will not be seen.
7. Teletherapy
When services are provided by teletherapy, the following apply:
The patient must be present and participating.
Sessions must be conducted in a quiet place of focus, not in a car or at the park or pool.
Sessions must take place within the State of California.
Showing up to a teletherapy appointment only to cancel, or failing to cancel before the start time, is considered a failure to attend.
8. Communication Consent
When you provide your contact information and set your communication preferences, you consent to receive text, email, and/or phone messages from us regarding appointment reminders, confirmations, surveys, and other communications specific to your or your child's care. Message frequency varies based on your enrollment and the reminders and alerts you select. You are expected to respond to appointment reminders to confirm or to cancel and reschedule, and we require at least 24 hours' notice for appointment changes. Please keep your contact information current so you continue to receive reminders. You may adjust your preferences at any time in your patient portal.
9. Fees and Payment
You are responsible for amounts not covered by your insurance, including copays, coinsurance, deductibles, and non-covered services.
Insurance and patient responsibility: We verify your benefits before your child begins. If your insurance denies a claim or does not pay for a service, our billing team will review the denial with you. This may include reviewing the Explanation of Benefits, participating in three-way calls with your insurance company, and pursuing appeals or reconsiderations where appropriate. If all available avenues are exhausted and the denial is upheld, the balance becomes your responsibility.
Hippotherapy technical fee: A technical fee applies to each hippotherapy visit. It is $30 per visit and increases to $35 per visit on November 1, 2026. The fee is paid directly to Friends of Leaps & Bounds at the time of service by Zelle (FOLB@LeapsPediatric.org), cash, or check. It is a non-covered service fee, not a copay, and is not eligible for insurance reimbursement.
Billing questions: Contact our billing team at billing@LeapsPediatric.org.
10. Personal Equipment
If you choose to leave personal equipment or assistive devices (such as AAC devices, iPads, gait trainers, or walkers) on our premises, you do so at your own risk. Leaps & Bounds is not responsible for the loss, theft, or damage of personal items left on-site. Please label all items with your child's name. Items are stored in a designated area that is not locked or supervised at all times, and we recommend taking equipment home after each session when possible.
11. Patient Portal and Accounts
Certain features, including the patient portal, require an account. You are responsible for keeping your login credentials confidential and for all activity that occurs under your account. Please notify us promptly of any unauthorized use of your account.

You agree not to misuse the website, attempt to gain unauthorized access to it or its systems, interfere with its operation or security, introduce harmful code, or use automated means to collect information from it without our permission. You agree to indemnify, defend, and hold harmless Leaps & Bounds and its affiliates from and against any claims, liabilities, damages, losses, and expenses, including reasonable attorneys' fees, arising out of or related to your use of our website or your violation of these Terms.
17. Changes to These Terms
We may update these Terms from time to time. Changes take effect when posted, along with an updated effective date. Your continued use of our website or services after changes are posted means you accept the revised Terms.
18. Governing Law
